Download the DOC and the Board of Pardons and Parole registration forms. Fill out the forms and mail them to the addresses provided on the forms. This process takes less than five minutes. If you prefer personal assistance with registration, call the DOC victims programs manager at (888) 223-6332.
Some victims wish to remain anonymous due to personal safety concerns. We take seriously your need for confidentiality and we do not share your registration information with offenders or others.* However, without your personal contact information , we cannot provide notification.
Notification Request Forms
* If a member of the public (including the offender) requests to see your registration information, a court will have to determine if your right to privacy outweighs the public right to know. DOC will defend your right of privacy unless you waive that right and allow the information to be disclosed.
